Who is Dr. John Provenzano? Dr. John Provenzano is a native of Wichita. As a graduate from Southeast High School, Dr. Provenzano went on to receive his degree in business/chemistry from Wichita State University in 1982, graduating Cum Laude. Dr. Provenzano attended UMKC School of Denistry and in 1986 graduated 4th in his class. He completed his residency at Charity Hospital in New Orleans in 1990, through the Oral & Maxillofacialsurgery program at Louisiana State University. Dr. Provenzano has extensive training in trauma repair, orthognathic surgery, jaw reconstruction, dental implants, office anesthesia and wisdom teeth removal. Scheduling an Appointment: To schedule an appointment please call us at (316) 681-3757 Office Location & Hours: 1515 S Clifton Suite 120 Wichita, KS 67218 Monday-Thursday 8 am - 4:30 pm Friday 8 am - 12 pm Please assist us at the time of your initial visit to the office by providing the following information: -Your referral slip and x-rays from your referring dentist -A list of medications you are currently taking -If you have dental insurance, please bring any forms of insurance cards with you to the appointment Please note: -All patients under the age of 18 must be accompanied by a parent or guardian at the consultation appointment. -Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves, or joints, heard murmurs requiring premedication, severe diabetes or hypertension -If your referring dentist has taken x-rays, you may request that they be forwarded to us Insurance Information: Our insurance coordinators deal with many different insurance companies. Most companies offer many different dental and medical plans. Some companies combine dental and medical coverage. These factors provide an environment for policies and guidelines that change weekly. This can make it almost impossible at times to accurately estimate our patientsâ€™ insurance co-payment. Many insurance companies will not give out fees until after the treatment is completed. Dealing with these companies can sometimes be difficult and time consuming. As a courtesy, we ask that you keep us informed of any change to your insurance. It is important that all information about you and your insurance be current. Patient Billing: For your convenience we accept Visa and Mastercard.
