Jason A. Brooks, M.D. received his Indiana State Medical License in April 2008. Dr. Brooks graduated with his Bachelors Degree from Purdue University in West Lafayette, Indiana in 1999 and obtained his Medical Degree from the Indiana University School of Medicine in Indianapolis, Indiana in May of 2004. Dr. Brooks completed his Post Graduate Training with a Transitional Internship at the Ball Memorial Hospital in June of 2005 and an Ophthalmology Residency at the Indiana University School of Medicine in Indianapolis, Indiana in June of 2008.
